Scheduling Appointments

To schedule an appointment, please call the office of the physician that you will be seeing. If you need to reschedule an existing appointment, please give at least 24 hours notice.

Your First Appointment 

Please plan to arrive 15 minutes before your scheduled appointment time to allow time to complete new patient paperwork and forms. We ask that you bring:

• Your current insurance card
• Any co-pay required by your insurance company
• A list of all medications (name and dosage) that you are currently taking 
• Copies of any relevant tests, such as X-rays and MRIs

We accept most insurance providers. Please contact your insurance company prior to your first visit to ensure coverage.
